A crane was used to tilt a steel cradle, which was attached on either side with slings. The slings were attached to clamps and the clamps were attached to the cradle. As the cradle was tilted, one of the clamps came loose and swung in the air, striking the back of an employee's head. He sustained a head laceration, requiring hospitalization and stitches. A hard hat was not worn at the time.
